Steps to Create a Feedback Form
1. Log in and Open the Event
- Log into your Indico account.
- From the dashboard, click National Research Conference 2025 (8 Sep – 13 Sep 2025) to open the event page.
2. Enter the Management Area
- On the event page, click the Management tab (gear icon in the top-right).
- In the left sidebar, scroll down and select Surveys [1].

3. Create a New Survey
- Click + Create survey [2] in the top-right corner.
4. Set Survey Details

- Title 2.1: Feedback Form – National Research Conference 2025
- Description 2.2: We value your feedback to improve future events. Please take a few minutes to complete this form.
- Access: Select Registered users only 2.3 (so only attendees can respond).
- Click Save.
5. Add Questions
- click on `Feedback Form – National Research Conference 2025' Manage [3] button

- Then Click on Prepare questinanarize [4] buttion

In the survey editor : "Add Question [5]"

Click Add question (+ icon) for each of the following:
- Text [9] – “What was the highlight of the conference for you?” (Required)
- Multiple Choice [7] – “How would you rate the event’s organization?” (Options: Excellent, Good, Average, Poor; Required)
- Scale /Numbers (1–5) [6] – “Rate the session quality (1 = Poor, 5 = Excellent)” (Required)
- Text [9] – “Any suggestions for future events?” (Optional)

Save each question before adding the next.
6. Preview the Form
- Click "Switch to the Display view [1]".

- in the slide bar Select "Servery [2]"

- Test the form by filling in sample answers, then discard the test.
7. Publish the Survey
- Click Open Nuw button.

- The survey will now appear as Open during the set availability period.
9. Monitor Responses
- During or after the event, go to Surveys → select your feedback form Manage → View responses.
